A young female patient came to me seeking a refined and feminine profile. Her primary aesthetic concern was dissatisfaction with the shape of her nose: a large nose with a prominent dorsal hump compromised the delicacy of her face and gave her features a harsher appearance. In addition, the patient noted concomitant nasal breathing difficulties, particularly during physical exertion.
Baseline Anatomical Features (Before Surgery)
During a detailed examination in profile and three-quarter views, I noted the following anatomical features:
- Nasal bridge: A pronounced, high osteocartilaginous hump that disrupted smooth lines and visually elongated the middle third of the face.
- Nasal tip: Tended to droop, with an almost absent well-defined transition from the bridge to the tip (supratip break). The nasolabial angle was excessively acute.
- Skin envelope: The patient’s skin was of moderate thickness. This is ideal for achieving a chiseled result; however, such skin requires extreme precision from the surgeon when rasping the bony nasal bridge, as even minor imperfections could become visible over time.
- Internal structures: A moderate deviation of the nasal septum and hypertrophy of the inferior nasal turbinates were identified, which was the underlying cause of impaired breathing function.
Surgical Course and Objectives
My objective was to make the nose neat, “weightless,” and completely restore free breathing while preserving a natural appearance. During the operation, I executed the following plan:
- Correction of aesthetic defects: I performed a careful reduction (removal) of the osteocartilaginous hump. I formed a perfectly smooth, elegant nasal bridge line with a subtle, barely perceptible transition to the tip.
- Tip contouring: I performed lateral cartilage reshaping, narrowed the nasal tip, and gently rotated (lifted) it upward. To secure the tip in this position and prevent future drooping, I used cartilage autografts.
- Functional stage: I performed septoplasty, straightening the septum and restoring full aeration of the nasal passages.
“After” Result
In the postoperative photos, we see a magnificent, stable result. The patient’s face has undergone a radical transformation, becoming incredibly delicate, harmonious, and attractive.
- The hump has been completely eliminated, and the profile line is now flawlessly smooth.
- The nasal tip was elevated, opening a beautiful, proper nasolabial angle, which visually reduced the overall length of the nose.
- Facial features appear softer and younger, and the gaze has become more open and expressive.
Nasal breathing function has been fully restored — the patient breathes easily and freely. She has freed herself from long-standing insecurities, feels wonderful, and is completely delighted with her new reflection.
