A young woman visited my clinic for a comprehensive rhinoseptoplasty. She was unsatisfied with a pronounced hump and felt her nose visually appeared too long and heavy, disrupting the overall delicacy of her facial features. Furthermore, during our consultation and examination, my suspicion of impaired respiratory function was confirmed—the patient regularly experienced nasal congestion, particularly at night.
Initial Anatomical Features (“Before” Photo)
During the three-quarter profile analysis of the face, I identified the following key anatomical features to address:
- A prominent osteocartilaginous hump that disrupted the smoothness of the nasal bridge and gave the profile an overly harsh appearance.
- An elongated, drooping nasal tip. Due to insufficient cartilaginous support, the tip pointed downwards, visually lengthening the middle third of the face and making the nasolabial angle overly acute. At rest, this gave the face a somewhat tired look.
- A deviated internal nasal septum, which was the root cause of the breathing difficulty.
- Medium-density skin. This skin type requires the surgeon to construct a reliable, sturdy cartilaginous framework so that the new, refined contours of the tip and bridge clearly define once swelling subsides and remain stable for a lifetime.
Surgical Procedure and Objectives
My primary goal was to achieve complete nasal harmonization—making it neat and feminine while preserving the patient’s unique individuality and avoiding an “operated” look.
The procedure was performed using a closed technique. First, I performed a septoplasty—gently straightening the nasal septum and clearing the nasal passages to fully restore normal internal aerodynamics.
Next, I moved to the aesthetic stage: delicately resecting the osteocartilaginous excess on the nasal bridge, completely eliminating the hump and forming a smooth, elegant line. After that, I worked on the alar cartilages: shortened the nasal length, gently elevated the tip (performed rotation), and securely fixed it in its new position. This allowed opening up the nasolabial angle and giving the tip a beautifully refined shape.
Surgical Result (“After” Photo)
In the “After” photos, we can observe a magnificent, stable result:
- The harsh hump has been replaced by an absolutely smooth, refined nasal bridge.
- The nasal tip became neat, elevated, and well-defined. Thanks to the reduction in nose length, the patient’s entire face was instantly transformed—appearing noticeably fresher, softer, and younger.
- The new proportions perfectly harmonize with the shape of her lips and chin, accentuating her natural beauty.
The patient was completely relieved of congestion problems—her nose now breathes perfectly freely. The result is harmonious, soft, and completely natural, bringing immense satisfaction to both myself and my patient.
