Male Profiloplasty: Elimination of a Massive Hump

Male rhinoplasty is always a delicate, precise work that requires preserving masculine features. A middle-aged patient turned to me for assistance. His main goals were removing a large dorsal hump, visually rejuvenating the face through tip correction, and, equally important, restoring proper nasal breathing, which had been impaired following an old sports injury.

Initial Anatomical Features (“Before” photos)

Upon analyzing the patient’s profile view, I defined the following treatment plan:

  • Nasal bridge: a massive, high osseocartilaginous hump that added excessive severity to the face and visually weighed down the middle third.
  • Nasal tip: under the force of gravity and weakened cartilaginous support, the tip had noticeably drooped downward. An acute nasolabial angle visually elongated the nose and made the face appear more tired, adding several years to the patient’s appearance.
  • Skin quality and structure: as with most men, the patient had relatively thick and dense skin. This required creating a strong, stable cartilaginous framework so that the new nasal contours would show clearly through the thickness of the soft tissues.
Surgical Course and Objectives

My main objective was to create a strong, straight, and completely natural masculine profile while simultaneously reconstructing the internal structures.

During the procedure, I performed a septoplasty — completely correcting the deviated portions of the septum and restoring the patency of the nasal passages. I then proceeded to the aesthetic stage: performing a careful reduction (lowering) of the bony hump while keeping the nasal bridge perfectly straight, without the slightest hint of a scoop. The nasal tip was delicately elevated (rotated) just enough to restore a harmonious male nasolabial angle (about 90–95 degrees) and securely fixed using cartilage grafts.

Surgical Outcome (“After” photos)

In the postoperative photos, we see a significant transformation of the profile:

  • Nasal bridge line: became impeccably straight and smooth, restoring balance between the forehead and chin.
  • Nasal tip: gained reliable support and stopped sagging, making the nose visually look more compact and neat.
  • Aesthetic outcome: the patient’s face looks noticeably fresher, younger, and lighter, while fully retaining its strong, masculine character. The quality of the surgery is confirmed by the completely natural appearance of the nose, leaving no visible signs of surgical intervention.

The patient is completely satisfied with the result: full, free breathing has been restored, and the refined profile has boosted his self-confidence.
