A young patient approached me, concerned about an aesthetic disproportion of her face. The main issue that bothered her was a large nose, which, according to her, dominated her other features and deprived her face of refinement.
Initial anatomical features (before surgery)
During visual analysis and palpation in profile, I noted the following features:
- Nasal dorsum: Presence of a high, rigid osteocartilaginous hump that sharply broke the profile line.
- Nasal tip: Noticeably drooped downwards, with insufficient projection. During smiling and facial expressions, the tip dropped even lower, which visually elongated the nose and gave the face an overly stern, tired appearance.
- Skin quality: Medium skin thickness with good elasticity, which allowed expecting quality tissue contraction after reducing nasal volume, but required precision accuracy from me when working with the underlying structures to prevent future contouring of the cartilages.
- Breathing function: The patient also noted periodic nasal congestion related to the deviation of internal cartilaginous structures.
Surgical course and objectives
My main objective was to create a natural, refined nose that would emphasize the beauty of the patient’s eyes without looking “operated on.”
During the closed rhinoseptoplasty, I performed:
- Careful removal of the osteocartilaginous hump and straightening of the nasal dorsum line.
- Reconstruction and correction of the deviated parts of the septum to fully restore free nasal breathing.
- Rotation (elevation) and delicate narrowing of the nasal tip, establishing a correct, stable projection. I securely fixed the tip in its new position so that it would not drop over time due to the action of facial expression muscles.
Result after 2 years
In the photo on the right, we see a stable, fully formed result 2 years after the surgical intervention. There is no trace of swelling left.
The line of the dorsum became smooth, soft, and perfectly straight. The nasal tip was elevated, opening up the correct nasolabial angle, which visually made the upper lip and chin more balanced. The nose harmonized perfectly with the overall proportions, the face began to look significantly softer, fresher, and younger, and the patient’s gaze opened up. Nasal breathing has been fully restored. The patient is completely satisfied with the result and feels absolutely confident.
