A young female patient of Caucasian appearance consulted me for nose shape correction. In addition to the external imbalance, she was concerned about nasal breathing difficulties, which often accompany such anatomical features. Our common goal was to create a graceful, natural-looking nose that would blend perfectly with her facial features without altering her individuality.
Initial Anatomical Features (“Before” Photo)
During the examination and profile analysis, I identified several key aesthetic and functional issues:
- A pronounced osteocartilaginous hump, which visually weighed down the middle third of the face.
- An elongated and drooping nasal tip. When smiling and at rest, the tip pointed downwards, which not only disrupted the nasolabial angle but also gave the face a more severe, tired, and visually older appearance.
- A deviated nasal septum. Asymmetry of the nasal bridge and axis in the frontal view indicated a deformation of the internal structures blocking normal airflow.
- Relatively thick skin. Working with such skin requires utmost precision from the surgeon, as it is less prone to “shrinking” and can conceal the delicate, precise work performed on the cartilaginous framework.
Surgical Course and Objectives
During the operation, I performed a closed rhinoseptoplasty. First and foremost, a septoplasty was carried out — I corrected the deformed nasal septum, restoring symmetry and full breathing.
Then I proceeded to the aesthetic stage: I carefully resected the hump, shaping a straight, smooth bridge line. I paid special attention to the nasal tip: through gentle modeling and partial excision of the alar cartilages, I managed to lift it, shorten it, and fix it in a proper, more youthful position. Given the skin thickness, the cartilaginous framework was reinforced so that the shape would remain stable over time and the tip would not droop.
Result After 3 Weeks (“After” Photo)
The photographs show an intermediate result — only 3 weeks have passed since the surgery. Despite the residual swelling that is natural for this period, the changes are evident:
- The nose became significantly shorter, the hump disappeared, and the bridge acquired correct and well-defined contours.
- The nasal tip is delicately lifted, opening up the correct nasolabial angle, which instantly refreshed the patient’s face and made it look younger.
- Symmetry in the frontal view has been restored, and the face has gained softness and internal harmony.
Most importantly, free nasal breathing has been completely restored. The young woman is incredibly pleased with the initial results, and as the swelling subsides over the course of the year, the nose will become even more defined and graceful.
