Introduction and Main Complaints
A charming young woman scheduled an appointment with the desire to achieve harmonious facial features and, just as importantly, to regain the ability to breathe freely. The primary reason for her visit was a noticeable nasal asymmetry, which not only disrupted the overall aesthetics but also caused the patient daily discomfort.
Initial Anatomical Findings
During the initial consultation, I thoroughly examined the baseline anatomical features. First, a pronounced deviation of the nasal axis and nasal septum was highly noticeable — this is exactly what caused the severe asymmetry of the nostrils and, consequently, serious difficulties with nasal breathing. Second, when analyzing the profile, I noted the presence of a hump and a slightly droopy, wide nasal tip, which visually weighed down the face. An additional challenge for me was the patient’s rather thick, porous skin. Such baseline data always require special meticulousness from the surgeon, since it is much harder to refine the elegant and delicate lines of a new tip underneath a thick skin envelope.
Surgical Progress and Technical Nuances
To comprehensively address both functional and aesthetic goals, I performed an open hybrid rhinoseptoplasty. It was an extensive and highly precise process. First and foremost, I performed a septoplasty: straightened the septum to restore full airflow and establish the correct central axis for the external nose. Given the skin thickness, I needed to create a strong and stable cartilaginous framework. I carefully removed the hump, lowering the projection of the nasal dorsum, and rotated the nasal tip upward, providing it with structure and proper support.
Result: Harmony and Functionality
The result of the work speaks for itself. The patient’s face was instantly transformed: the heaviness disappeared, her gaze became more open, and her features acquired softness and refinement. We achieved an ideally straight, aesthetically precise nasal dorsum with a smooth transition to a neat tip. The asymmetry is completely eliminated — the proportions of the nostrils are now harmonious. And the most important achievement of this operation is that my patient not only admires her new, elegant profile in the mirror, but can finally breathe deeply.
