A young woman scheduled an initial consultation complaining of a pronounced aesthetic disproportion: a large, masculine nose visually weighed down her delicate features and deprived her look of femininity. In addition, due to the deviation of internal nasal structures, she periodically experienced difficulties with full nasal breathing.
Initial Anatomical Features (“Before” Photos)
During the evaluation of the profile view, I noted a complex of challenging anatomical nuances that required high-precision, meticulous work:
- Nasal dorsum: there was a pronounced, high osteocartilaginous hump that disrupted the smoothness of the profile lines.
- Nasal tip: was noticeably droopy (pollybeak deformity), had excessive length, and shifted even lower when smiling, which visually aged the patient and gave her face a somewhat tired appearance.
- Skin: rather thick in the tip area, which always requires utmost precision from the surgeon when shaping new contours to ensure that subcutaneous changes do not “blur” over time.
Surgical Progress and Goals
My primary goal was not merely to reduce the size of the nose, but to completely harmonize it in accordance with the facial proportions, while preserving the young woman’s individuality.
During the surgery, I performed a careful resection of the osteocartilaginous hump, shaping a smooth and elegant nasal dorsum. Special attention had to be paid to the tip: I shortened the lateral cartilages, elevated the tip, and securely fixed it in a new, anatomically correct position. Given the skin thickness, I needed to create a well-defined, structural cartilaginous framework so that the refined tip would look neat and contoured. Concurrently, a septoplasty was performed to straighten the septum and restore unobstructed breathing.
Result after 6 Months (“After” Photos)
At the follow-up examination six months later, we can evaluate an interim but already stable result. The swelling has almost completely subsided, revealing delicate and well-defined lines.
We managed to achieve a stunning effect:
- The nasal dorsum became perfectly smooth, with a beautiful transition to the lateral vaults.
- The nasal tip was elevated, the excess length was eliminated, and the profile itself began to look soft and natural.
- The patient’s face was instantly transformed — her eyes opened up, and her features began to look significantly younger, fresher, and softer.
Most importantly, comfortable nasal breathing has been completely restored. The young woman is incredibly pleased with the result, smiles openly, and feels absolutely confident.
