Removal of a large hump and creation of elegant facial proportions

A young woman came for an initial consultation with a clear request for nose shape correction. A large, dominant nose with a high hump disrupted the natural balance of facial features, visually weighed down the middle third, and made the profile excessively harsh. As in most cases with such anatomy, aesthetic complaints were accompanied by functional ones: the patient noted difficult nasal breathing due to a deviated internal septum.

Preoperative Anatomical Analysis (“Before”)

During a detailed preoperative assessment of the patient’s profile, I noted the following features:

  • Nasal dorsum: A high, prominent osteocartilaginous hump that created a sharp angle and made the nose look visually too long.
  • Nasal tip: It was noticeably droopy, making the nasolabial angle excessively acute. During facial expressions, this effect was intensified, giving the face a tired look.
  • Skin quality: The patient’s skin is rather thin. From a surgical standpoint, this requires utmost, jewel-like precision: thin dermis is unable to hide even the slightest irregularities of the underlying framework. Every movement of the instrument during bone and cartilage reshaping must be absolutely precise.
Surgical Objectives and Procedure

My main goal was to restore softness, harmony, and pronounced femininity to the face, while avoiding the effect of an “artificial” or cookie-cutter nose.

During the surgery, I performed a careful reduction of the osteocartilaginous part of the nasal dorsum, completely eliminating the hump and forming a smooth, elegant line. The next crucial step was working with the tip: I performed a delicate upward rotation of the nasal tip and adjusted its projection, opening up the nasolabial angle to ideal female parameters (around 100–105 degrees). As an internal step, septoplasty was performed — I straightened the nasal septum, fully restoring free and full breathing.

Result 1 Year Post-Operation

The “1 Year After” photograph clearly demonstrates a stable, fully formed result:

  • Graceful contours: There is no trace left of the former massive hump. The profile line has become flawlessly smooth, even, and refined.
  • Rejuvenating effect: The new, slightly elevated position of the nasal tip instantly transformed the facial expression, making it look significantly softer, more open, fresh, and visually younger.
  • Harmony of proportions: The new, neat nose perfectly balanced the line of the forehead and chin, highlighting the natural beauty of the patient’s lips.

A year later, the thin skin has flawlessly contracted over the new, sculpted framework. My patient has completely freed herself from her previous insecurities, gained absolute self-confidence, and, just as importantly, enjoys easy and free nasal breathing.
