A young and very beautiful girl came to the consultation with a complex request — she needed a secondary rhinoseptoplasty. For me as a surgeon, such cases always require special attention and empathy: the patient already has a negative experience, aesthetic disappointment, and, as a rule, seriously compromised anatomy. My main goal was not only to give the girl the nose of her dreams but also to restore lost trust, as well as to rebuild full nasal breathing.
Initial anatomical features
Carefully studying the “Before” picture, I noted the consequences of the previous intervention. The nasal dorsum retained an uneven contour with a residual hump, and the tip was under-defined, wide, and slightly drooping, which visually weighed down the lower third of the face and gave a tired look. There was also noticeable asymmetry and deviation of the nasal septum, causing the patient constant difficulties with free inhalation. The situation was slightly complicated by the skin quality: it is quite thick and porous, which always requires me to create the most defined and rigid internal framework so that the elegant shapes are not “lost” under the weight of the tissues.
Surgical process
The surgery was meticulous. Secondary intervention is always working in conditions of altered anatomy and scar tissue. I performed a complete reconstruction: carefully dismantled the old structures, straightened the septum (performing a thorough septoplasty to restore functionality), and rebuilt the cartilaginous support. Jeweler-like precision work with the nasal tip allowed me to set the correct rotation and narrow it, overcoming the resistance of the thick skin.
Result
In the “After” shots, taken immediately upon completion of the surgery, the result of my work is visible. I formed a perfectly straight, noble dorsum with a soft, feminine transition to the tip. The tip itself became chiseled, neat, and slightly upturned. These changes radically transformed the patient’s face: it began to look much fresher, younger, and more harmonious. I not only achieved a beautiful aesthetic silhouette but also fully restored the breathing function. The patient is truly happy, and for me, there is no greater reward than seeing this delight from the result.
