A patient in need of secondary rhinoseptoplasty turned to our clinic for help. Revision surgeries are always a special challenge for a surgeon, requiring not only technical mastery but also a deep understanding of altered anatomy. The young woman was upset with the results of her previous surgery: she was dissatisfied with both the aesthetics and the quality of her nasal breathing.
Initial anatomical features
Carefully studying the initial “Before” picture, I noted a number of problems. The nasal tip lacked structural definition, appeared massive, slightly asymmetrical, and under-projected, which weighed down the lower third of the face. Irregularities in the contour were also visible on the nasal dorsum — frequent companions of previous interventions. In addition, the presence of a pronounced deviated septum and adhesions prevented my patient from breathing fully and freely, making functional correction absolutely necessary.
Course of the surgery
During the operation, I encountered typical difficulties of secondary rhinoplasty: the presence of dense scar tissue and a deficit of native cartilaginous structures. My task was to perform truly jewelry-like precision work — to delicately excise scars, restore a reliable supporting framework of the nose, and recreate correct, elegant proportions while working with moderately thick skin. I performed a full septal reconstruction, restoring physiological health to the airways.
Result
In the “After” footage, taken right on the operating table (immediately after placing the finest sutures on the columella), the stunning result of my work is visible. I formed a neat, structural, and slightly elevated nasal tip, and created a flawlessly straight and elegant dorsal line. The nose became refined, harmonious, and symmetrical. Even taking into account the inevitable intraoperative tissue swelling, it is obvious how much the face has transformed — it has become more delicate, open, and fresh. I am happy that I was able to restore not only the patient’s beauty but also her self-confidence, and most importantly — free breathing.
