This patient came to me with a desire to correct the shape of her nose, which, in her opinion, disrupted the harmony of her face. The main factor was a large, high hump on the nasal bridge, as well as a noticeably drooping tip. In profile, this anatomical feature visually weighed down the middle third of the face, making the look overly strict and adding age.

Upon examination, I identified several key objectives that required precise surgical workmanship:
- Nasal bridge: The presence of a pronounced osteocartilaginous hump that disrupted the smoothness of the profile line.
- Nasal tip: Drooping, with insufficient projection and an acute nasolabial angle. When smiling or during facial expressions, the tip dropped even lower, intensifying the visual imbalance.
- Skin: The patient’s skin is quite thick, which always places a special responsibility on the surgeon. Thick skin is reluctant to contract and can smooth out the fine contours of the cartilaginous framework, so working with the nasal tip required maximum precision.
Course of the operation and surgical tasks
My main goal was not just to “reduce” the nose, but to completely harmonize the facial features while preserving the patient’s individuality.
During the surgery, I carefully removed the osteocartilaginous hump, creating a smooth, elegant nasal bridge. I paid special attention to the tip area: I performed rotation and delicately lifted the nasal tip, securing it in a correct, anatomically advantageous position. This allowed us to open up the nasolabial angle and make the profile softer. Considering the thickness of the skin, I modeled the cartilaginous framework in such a way that the new contour stood out clearly while remaining natural.

At the follow-up examination six months later, we can observe an excellent intermediate result (although the soft tissues continue to heal).
We managed to achieve a perfect balance: the nasal bridge became completely straight, and the neatly lifted tip gave the face openness, lightness, and a noticeable rejuvenating effect. Profile looks natural, the lines are smooth, and the nose itself no longer distracts from the beauty of the patient’s eyes. The new reflection in the mirror fully lived up to my patient’s expectations, and I am sincerely glad to see her happy smile.
