A young girl approached me with a typical concern — aesthetic dissatisfaction with her nose shape and the presence of a dorsal hump. Visual and palpation examination in profile clearly revealed a pronounced bony-cartilaginous hump that visually elongated the nose. The nasal tip lacked projection and was slightly drooping downward, which, combined with the hump, created a sense of heaviness in the middle third of the face and visually aged the patient.
Initial Clinical Presentation
- In full face, there was moderate asymmetry of the dorsum and the nasal tip, as well as a widening of its base.
- The patient’s skin was of medium thickness and quite elastic, which allowed for good tissue redraping in the postoperative period; however, it required surgical precision from me when working with the underlying cartilaginous structures to prevent any contour irregularities.
- In addition to the aesthetic aspect, the examination revealed a deviated nasal septum, which caused difficulty in free nasal breathing.
Objectives and Surgical Course
My main goal was to perform a comprehensive rhinoseptoplasty: to restore full breathing function and create a harmonious, elegant nose silhouette that would perfectly fit the overall proportions of the face while preserving the patient’s individuality.
During the surgery, I performed the following steps:
- Functional stage (Septoplasty): Corrected the nasal septum deformity, restoring the patency of the airways.
- Dorsum correction: Carefully modeled the bony and cartilaginous sections, completely removing the hump and forming a straight, refined dorsal line.
- Nasal tip rhinoplasty: Rotated the nasal tip into the correct anatomical position (slightly lifted it) and narrowed its cartilages, reducing excess volume and giving this zone a distinct definition.
- Base narrowing: Approximated the nasal bones (osteotomy) to narrow the bridge in full face, making the nose more petite.
Results after 6 months
At the follow-up examination six months later, we see a stable and natural result. The nose has become significantly more elegant and proportionate. The profile line is now completely straight and smooth, and the lifted, neatly defined tip has given the eyes an open look and the entire face freshness, softness, and youthfulness.
In full face, the nose looks symmetrical, neat, and no longer draws undue attention to itself. Free nasal breathing is fully restored. My patient is incredibly pleased with her changes: the renewed profile has increased her self-confidence, and her facial features have achieved long-awaited harmony.
