Another clinical case began with a visit from a vibrant young woman with expressive facial features. Her main aesthetic complaint concerned the shape of her nose: a large hump and a drooping tip made her profile look overly strict, heavy, and visually added years to her age. Furthermore, a detailed examination confirmed my functional suspicions — a deviation of the internal nasal septum made free nasal breathing difficult.
My task as a surgeon was not just to eliminate anatomical flaws, but to perform a delicate, artistic work — softening the facial features, making the nose neat and feminine, while fully preserving the patient’s individuality and natural magnetism.
Anatomical Analysis “Before”:
- Nasal dorsum: a high, well-defined osteocartilaginous hump was identified. In the frontal view and basal projection, a slight deviation (displacement) of the dorsum to the side was noticeable.
- Nasal tip: dense, significantly drooping downwards. An acute nasolabial angle visually elongated the central third of the face and deprived it of lightness. Asymmetry of the alar cartilages was also present.
- Internal structures: pronounced deviation of the nasal septum, leading to impaired aerodynamics and chronic breathing deterioration.
Surgical Procedure and Correction Nuances:
The operation was performed using a closed rhinoseptoplasty approach. At the first, functional stage, I corrected the deformed septum, clearing the airways. Then, I proceeded to model the new profile: carefully resected the hump, forming an absolutely smooth, elegant line of the nasal bridge. The tip required special attention — I performed its rotation (lifted it upwards) and established the correct projection. Working with the cartilaginous structures of the tip required utmost precision to achieve perfect symmetry of the nostrils and a smooth transition from the dorsum to the tip.
Result “After” (6 Months Later):
In the photographs, we see the result 6 months after the surgery. By this time, the main post-operative swelling had already subsided, revealing a graceful and chiseled silhouette.
- The hump has been completely eliminated — the nasal dorsum has become flawlessly straight and harmonious.
- The nasal tip is delicately lifted and narrowed; the nasolabial angle now corresponds to the aesthetic canons of female attractiveness.
- The excess length of the nose is gone, which instantly refreshed the face, making it look softer, younger, and more delicate.
- Complete symmetry of all nasal elements has been achieved, both in the frontal view and from the base (basal view).
The patient’s face sparkled with new colors, her look became more open, and her smile more confident. Nasal breathing was fully restored. My patient is happy, and for me, her smile and shining eyes are the main criteria for a successfully completed work.
