A young woman approached me for a comprehensive reshaping of her nose. Her primary aesthetic concerns were a large dorsal hump and the overall massiveness of the nose, which disrupted the balance of her beautiful and delicate facial features. Furthermore, a detailed anatomical analysis confirmed functional issues as well—a deviated septum significantly obstructed free nasal breathing.
Initial Anatomical Presentation (Before Surgery)
Analyzing the preoperative photographs, I identified several complex challenges that required meticulous surgical work:
- Nasal dorsum: In the three-quarter view, a large osteocartilaginous hump was clearly visible, depriving the profile of femininity.
- Nasal tip and base: The basal view (bottom view) clearly demonstrated a pronounced asymmetry of the alar cartilages and nostrils caused by the septal deviation. The tip itself was quite wide, bulbous, and slightly droopy, making the middle third of the face look heavy.
- Skin quality: The patient’s skin is quite thick in the tip area, which imposed special obligations: to achieve a well-defined and elegant result, the new cartilaginous framework had to be modeled perfectly so that the contour would not become blurred during the rehabilitation process.
Surgical Course and Nuances
During the surgery, I performed a closed rhinoseptoplasty, combining aesthetic correction with the restoration of the patient’s health:
- Functional stage: I performed a septoplasty, eliminating the nasal septal deviation. This allowed not only restoring symmetry to the base but also completely restoring full nasal breathing.
- Work on the dorsum: I delicately resected the hump and performed an osteotomy to narrow the bony framework and make the nasal dorsum straight, refined, and harmonious.
- Tip remodeling: A major reconstruction of the nasal tip cartilages was performed. I eliminated the asymmetry, narrowed the tip section, and slightly rotated it upwards, setting a correct, aesthetically precise nasolabial angle.
Result after 9 Months
At the follow-up examination 9 months later, we see a stunning transformation. The swelling has almost completely subsided, revealing an elegant and stable result:
- The profile has become soft and neat, with an absolutely straight dorsal line.
- When viewed from below, the restored symmetry of the nostrils and a beautifully defined, narrowed nasal tip are clearly visible.
- The patient’s face has gained refinement, her gaze has become more open, and the overall look is incredibly gentle and harmonious.
The patient has completely gotten rid of nasal drops, breathes easily and freely, and her happy smile is the best evaluation of my work.
