A young man came in for a consultation with a classic request for profile harmonization and the improvement of nasal breathing. Working with male faces is always a special responsibility and a meticulous search for balance: it is necessary to correct defects while strictly preserving masculine features, avoiding the slightest hint of over-feminization or a “done” look to the nose.
Initial Anatomical Features
During the “Before” examination stage, I carefully analyzed the initial anatomy. A pronounced osteo-cartilaginous hump and a somewhat massive, broad nasal tip, which visually weighed down the lower third of the face, immediately caught the eye. The patient’s skin is quite thick — this is a typical male characteristic that always complicates the surgeon’s task, as it requires creating a very strong and stable cartilaginous framework to form an elegant result. In addition, the diagnosis revealed a deviated nasal septum, causing the patient to experience significant difficulties with proper breathing.
Surgical Process
I performed an open hybrid rhinoseptoplasty. My main goal was to solve two problems simultaneously — aesthetic and functional. I delicately lowered the nasal dorsum, completely removing the hump, and performed a structural tip rotation, making it more defined and neat. In parallel, I performed a septoplasty, straightening the septum to return the joy of a free breath to my patient. When working with thick skin, I paid special attention to the tip projection so that it does not drop over time and maintains its set shape.
Result
In the “After” footage, filmed right on the operating table, you can evaluate our result. We achieved a straight, noble, and strong masculine dorsum. The nasal tip became proportional, without excessive upturn, which would have been completely inappropriate here. The patient’s face instantly transformed, becoming more open, fresh, and harmonious, while preserving its individuality and ruggedness.
Breathing functionality has been fully restored, and the aesthetic outcome exceeded all expectations. And, as a funny quote from our video says: “The eyes are mom’s, but the nose is from Gevorg Yuryevich.” It is a great pride for me to leave such a beautiful author’s signature on the happy faces of my patients.
