A young man entered the office with a classic request combining the need for aesthetic harmonization and functional treatment.

Initial Anatomical Features

During a detailed visual analysis of the initial “Before” picture, I noticed a pronounced osteo-cartilaginous hump and a rather massive, drooping nasal tip. Such architecture visually weighed down the features, giving the face a sterner and more tired appearance. In addition, there was an obvious deformation and deviation of the nasal septum — a sure sign that the patient had been experiencing severe difficulties with full nasal breathing for a long time. An additional challenge for me was the thick, porous male skin. Working with this type of tissue requires a particularly meticulous and structural approach: so that the refined nasal tip would not get lost under the thickness of the skin, I needed to create the most rigid and reliable supporting cartilaginous framework.

Surgical Procedure

To address all these tasks, I performed an open hybrid rhinoseptoplasty. During the operation, I straightened the septum, completely restoring the patency of the airways. Then I delicately removed the hump, formed a smooth, straight dorsum, and elevated the nasal tip, giving it an anatomically correct angle without depriving the profile of its masculine character.

Result

Comparing the “After” result, one can notice how much the perception of the face as a whole has changed. The profile became noble, smooth, and harmonious. The face lost its former heaviness, the gaze became more open and fresh, and most importantly — my patient can now breathe freely. As we joked with him while summarizing the transformation: “The eyes are from mom, but the nose is now from Gevorg Yuryevich.” Seeing the patient’s sincere joy and confidence is the best reward for my work.
