This clinical case holds a special place in my practice — it is my seventh charity surgery for displaced persons from Artsakh. A young man sought a consultation, requesting not only aesthetic correction but also an improvement in his quality of life, as the initial nasal anatomy clearly indicated breathing problems.
Initial Anatomical Features
Upon detailed examination of the “Before” face, I noticed pronounced asymmetry and deviation of the nasal axis. The dorsum was visually widened, and the nasal tip was massive, bulbous, and slightly droopy, which made the facial features heavier. In addition, I noted rather thick, porous skin, typical of the male face type. Such tissue structure always complicates the surgeon’s task, as it is much harder to demonstrate a refined and defined cartilage contour under thick skin, and a massive tip tends to droop over time. Due to a septal deviation, the patient also experienced severe difficulties with nasal breathing, which required mandatory functional intervention.
Course of Surgery
During the surgery, my goal was to perform a comprehensive septorhinoplasty.
- I performed a septoplasty to straighten the septum and fully restore free nasal breathing — this is the fundamental basis of health.
- I carried out meticulous work with the cartilaginous framework: narrowed and straightened the nasal dorsum, and also carefully reconstructed the tip. To overcome the resistance of the thick skin, I created a strong cartilaginous support, raising the tip to a physiologically correct angle and giving it a neater, yet absolutely masculine shape.
Result
The “After” result exceeded our expectations. The face acquired correct, harmonious proportions, the nose became straight, neat, and aesthetically pleasing, while retaining its natural masculinity. The patient breathes freely and fully. As we smilingly noted after the surgery: “The eyes are his mother’s, but the nose is from me.” I am sincerely proud that I could help this young man start a new chapter of his life with confidence and comfort.
