Initial Clinical Picture

A young man consulted me for the beautification of his male profile. He was concerned about a prominent dorsal hump and a drooping nasal tip, which gave his profile an overly harsh and disharmonious appearance. As is often the case in male rhinoplasty practice, aesthetic discomfort was accompanied by functional problems: the patient noted persistent difficulty with nasal breathing, worsening during sleep and physical activities.

During the examination and analysis of the “Before” profile, I identified the following anatomical features:

  • Nasal dorsum: A large, angular osteocartilaginous hump was identified.
  • Nasal tip: Had a tendency toward drooping (ptosis), forming an acute, closed nasolabial angle. The nose visually appeared excessively long and overhanging the upper lip.
  • Internal structures: Rhinoscopy revealed severe deviation of both the cartilaginous and bony parts of the nasal septum, disrupting airflow dynamics.
  • Skin: Fairly thick, typical for men, which required me to create a stable, distinct cartilaginous framework so that the new nasal contours would not “blur” beneath the soft tissue layer during the healing process.
Objectives and Surgical Procedure

Male rhinoplasty demands a special, delicate approach and a deep understanding of gender architectonics. My main objective was to eliminate aesthetic defects and restore free nasal breathing, while strictly preserving masculine facial features. An excessive slope of the dorsum or an overly upturned tip is strictly unacceptable here.

I performed a combined rhinoseptoplasty. First, I corrected the septal deviation, fully restoring nasal breathing function. Then I proceeded to aesthetic modeling: carefully resecting the osteocartilaginous hump and performing meticulous rasping to form a completely straight, strong masculine dorsum. The nasal tip was delicately rotated upward and securely fixed at an angle of approximately 90–95 degrees, which is the aesthetic gold standard for men. We removed the excess length of the nose, making the facial proportions harmonious.

Surgical Result

In the “After” photo, we see a result that clearly demonstrates how targeted nasal reshaping alters the perception of the entire face, making it more open and appealing. The heavy hump has been completely eliminated, and the dorsal line has become perfectly smooth and defined. The nasal tip no longer droops; it is elevated just enough to lighten the profile while keeping it masculine and strong-willed.

The new nose perfectly balanced the middle third of the face, harmonizing with the chin line and emphasizing correct male proportions. Complete restoration of nasal breathing allowed the patient to forget about congestion forever. Both the patient and I were completely satisfied with the result of the work done.
