Another clinical case began with a visit from a young woman who was aesthetically dissatisfied with her nasal profile. The prominent, rigid contour of her nose clashed with her soft facial features, distracting attention from her eyes and making her overall expression appear stricter than it actually was.
Initial Anatomy (“Before” photo)
When analyzing her profile, I identified several key anatomical features that required surgical correction:
- Nasal bridge: A pronounced, high osteocartilaginous hump that disrupted the smoothness of the lines and visually elongated the middle third of the face.
- Nasal tip: Slightly drooped, with insufficient projection and an acute nasolabial angle. During facial expressions, the tip pulled downwards, adding heaviness to the lower third of the face.
- Tissue quality: The patient had fairly elastic yet thick skin in the tip area, which always requires the surgeon to build a reliable, stable cartilaginous support so that the new delicate contours do not blur during the healing process.
Surgical Goals and Procedure
My main objective was to reduce the excess volume while maintaining absolute naturalness of proportions, avoiding the “operated nose” look. The face needed to become softer, more feminine, and more harmonious.
During the surgery, I performed a careful reduction of the dorsum projection, completely removing the osteocartilaginous hump and creating a smooth, refined line. Next, I moved on to the most crucial stage — modeling the nasal tip. I performed a delicate upward rotation, lifting and securing it in a new, aesthetically ideal position. This beautifully widened the nasolabial angle and visually shortened the nose, making the profile light and balanced.

In the “After” photos, we see a magnificent result of the transformation:
- The hump is completely removed, and the bridge line has become perfectly straight and elegant.
- The nasal tip is lifted, gaining defined contours and proper projection.
- The new proportions of the nose harmonize perfectly with the patient’s chin and lip lines.
The patient’s face was completely transformed: the former severity disappeared, her gaze became more open, and a noticeable effect of rejuvenation and lightness emerged. The nose looks as if it were natural from birth — which is the highest praise for a plastic surgeon’s work. The patient is completely delighted with her new look, and her happy smile is the best reward for me.
