A young woman approached me seeking both aesthetic and functional correction. A prominent hump and a severely drooping tip made her profile look overly harsh and masculine, visually adding years to her age and giving her face a tired expression. Furthermore, the patient complained of intermittent nasal breathing difficulties.
My primary goal was to achieve complete facial harmonization: remove the heavy hump, shorten the nose, and lift the tip, thereby rendering the lines smooth, feminine, and natural.
Anatomical Analysis “Before”:
- Nasal dorsum: a massive, high osteocartilaginous hump was present. In the frontal view, the dorsum appeared excessively wide and showed a slight deviation from the midline axis.
- Nasal tip: significantly drooping downward (tip ptosis), poorly projected, and wide. The acute nasolabial angle visually elongated the face and added heaviness to the upper lip area during facial expressions.
- Skin quality: medium-thickness skin, which required highly precise work on the cartilaginous structures to ensure that the new nasal contour would manifest clearly and elegantly once the swelling subsided.
- Function: deviation of the internal septal structures disrupted the proper distribution of airflow.
Surgical Procedure and Objectives:
During the closed rhinoseptoplasty, I addressed both functional and aesthetic aspects simultaneously. First, I performed a septoplasty, straightening the nasal septum and restoring proper breathing. Next, I proceeded with the reshaping phase: carefully resecting the hump and performing a precise osteotomy to narrow the nasal dorsum and refine its lines.
Special attention was paid to the nasal tip. I shortened the caudal aspect, rotated the tip upward to establish a new, anatomically correct, and aesthetically pleasing position, and securely stabilized the cartilages.
“After” Result:
The postoperative photographs demonstrate a beautiful, stable corrective outcome. The patient’s face has undergone a remarkable transformation:
- The heavy hump has been completely eliminated — the profile now features an absolutely straight, elegant line.
- The nasal tip is lifted, creating a beautiful, open nasolabial angle. The nose now appears refined and well-proportioned.
- Correcting the excessive nasal length instantly “opened up” the face, making the eyes more expressive and giving the overall appearance a significantly younger, softer, and fresher look.
- Nasal breathing has been fully restored.
Thanks to the surgery, we successfully relieved the patient of her long-standing insecurities. While the face beautifully preserved its unique individuality, it now looks refined and harmonious, allowing the young woman to regain absolute confidence in her appearance.
