A young female patient approached me to harmonize her profile and add softness and femininity to her face. Her primary aesthetic concerns related to the shape of her nose: the presence of a hump and a visually drooping tip, which, in her view, weighed down her face and deprived it of refinement.
Initial Anatomical Features (“Before” Photo)
When evaluating the patient’s profile, I identified several key nuances that determined the course of the surgery:
- A pronounced osteocartilaginous hump that disrupted the smoothness of the nasal bridge and made the profile excessively harsh.
- A drooping nasal tip. Due to a lack of cartilaginous support, the tip dipped downward, narrowing the nasolabial angle. This visually elongated the nose and gave the face an overly strict, tired, and even slightly aged appearance.
- Thin skin. This is a crucial factor for a plastic surgeon. Thin skin is an excellent tool that allows for ideal definition and a sculpted tip. However, it is completely unforgiving of mistakes: any defect or irregularity of the underlying cartilaginous framework will become visible over time. This work demanded absolute, jeweler-like precision from me.
Course of the Operation and Surgical Objectives
My primary objective was not just to remove the hump, but to build an absolutely stable nasal architecture that would not deform over time.
I performed a closed rhinoplasty. In the first stage, I carefully resected the osteocartilaginous excess, restoring a smooth and natural line to the nasal bridge. Next, I moved on to delicate modeling of the tip: I gently lifted it, created a beautiful projection, and opened the nasolabial angle. To protect the nose from age-related drooping caused by gravity, I applied a structural reinforcement technique to the cartilaginous framework. All cartilage contours were meticulously smoothed so that the result would look flawless under the thin skin.
Result After 5 Years (“After” Photo)
The second image in this collage is a matter of my special professional pride. The photograph was taken exactly 5 years after the surgery. We can see an ideal, time-tested result:
- The nasal bridge remained perfectly smooth, without any sagging or bone overgrowth.
- The nasal tip maintained its pre-set height, elegance, and delicate shape, without dropping even a millimeter, which proves the reliability of the cartilaginous framework I created.
- The nasolabial angle became harmonious, the profile looks soft, and the patient’s face looks noticeably refreshed and visually younger.
The five-year result clearly demonstrates that a competently performed rhinoplasty is a once-in-a-lifetime surgery. The patient is absolutely happy and enjoys her natural, harmonious look.
