A young woman came to the consultation with a pronounced aesthetic and functional request. She was concerned about a noticeable facial asymmetry caused by severe nasal deviation, as well as a heavy visual imbalance—the large nose disrupted the harmony of her naturally delicate and beautiful facial features. In addition, due to the deviation of internal structures, the patient suffered from constant difficulty in nasal breathing.
Initial anatomical data (“Before” photos)
Upon a detailed facial analysis, I identified a complex of combined issues that required a radical yet careful restructuring:
- Axis deviation: The frontal view clearly shows that the nasal pyramid is significantly shifted to the side, creating a pronounced S-shaped asymmetry and deviating the septum.
- Nasal bridge: The oblique view revealed a large, rigid osteocartilaginous hump that visually weighed down the middle third of the face.
- Nasal tip: It was bulbous, wide, and drooping downward, giving the face an overly stern and tired appearance.
- Skin: Fairly thick in the lower third of the nose. This type of skin requires the surgeon to create a very well-defined, stable cartilaginous framework; otherwise, the newly refined contours might simply become “blurred” during the healing process.
Surgical course and objectives
Correcting a pronounced deviation is always a challenge for a rhinoplasty surgeon. My work was conducted simultaneously in both functional and aesthetic directions.
As a first step, I performed a septoplasty—aligning the deviated cartilaginous and bony parts of the nasal septum to completely clear the airways. Then I moved on to the external reconstruction: carefully removing the hump and performing a controlled osteotomy to return the nasal bridge strictly to the central position (the midline of the face) and narrow it. The nasal tip required complex reshaping: I narrowed the alar cartilages, modified their configuration, and gently lifted the tip, securely fixing it in its new position. This allowed for the creation of a beautiful, chiseled contour that became clearly visible even through the thick skin.
Result after 1 year (“After” photos)
The photos were taken exactly one year after the surgery. This is precisely the timeframe when the final, stable, and fully formed result can be evaluated—all hidden swelling has subsided, and the tissues have completely adapted to the new framework.
Look how the patient’s face has been transformed:
- Perfect symmetry: The nose is now located strictly in the center of the face, and the asymmetry is completely eliminated.
- Elegant profile: In place of the hump, a straight, graceful bridge line has appeared, with a beautiful, smooth transition to the tip.
- Refined tip: It has become neat, defined, and slightly upturned, which instantly made the appearance more noble, feminine, and youthful.
- Harmony and function: The nose no longer draws all the attention, harmoniously accentuating the beauty of the young woman’s eyes and lips. But most importantly, my patient breathes absolutely freely and feels incredibly confident.
