Aesthetic Transformation and Refinement of the Delicate Tip

A young woman came to me with a classic aesthetic request. The patient really wanted to get rid of a large hump that deprived her profile of femininity and to make her nose more refined. In addition, due to a deviated internal septum, she regularly experienced nasal congestion, which reduced her quality of life.

Initial Anatomical Features (“Before” Photos)

When analyzing the face in profile (both angles are clearly visible in the photos), I noted the following features:

  • Nasal dorsum: a high, pronounced bony-cartilaginous hump was identified. It created a sharp, harsh contour and visually weighted down the entire middle third of the face.
  • Nasal tip: was excessively long and drooping downwards. Due to an acute nasolabial angle, the face appeared more stern, and the profile acquired a visually tired look.
  • Skin quality: in the tip area, the skin was quite thick and soft. For a surgeon, this is always a sign that targeted work is required—creating a sturdy cartilaginous framework that can clearly define itself through the tissue thickness.
Surgical Procedure and Surgical Goals

My goal was to create a harmonious, natural nose that enhances the young woman’s natural beauty without overpowering her features.

First, I performed a septoplasty—correcting the deviated sections of the septum to fully restore unimpeded airflow. Then, I proceeded with open rhinoplasty: performed a gentle reduction of the hump and narrowed the nasal dorsum using delicate osteotomy.

The most crucial stage was modeling the tip. I shortened its projection, elevated the cartilaginous structures, and created a beautiful, open nasolabial angle. The thick skin required stable support, so I securely fixed the tip in its new position using autografts.

Correction Result (“After” Photos)

In the postoperative photos, we see a magnificent, stable result. The nose has fully adapted to the new facial proportions:

  • Ideal profile: in place of the harsh hump, there is now a smooth, elegant dorsum with a delicate transition to the tip.
  • Harmonious tip: it has become refined, structured, and slightly elevated. The young woman’s face immediately refreshed, and her gaze became more open and gentle.
  • Natural appearance: the nose looks completely “native,” harmonizing with the shape of the lips and chin.

My patient completely got rid of her insecurities, her breathing became perfectly free, and her smile in the “After” photo is the best confirmation of a successfully performed job.
