A young woman came to my appointment with a combined aesthetic and functional problem. Her main complaint was a severe, visible nasal deformity that critically disrupted her facial harmony en face, as well as a constant, pronounced difficulty with nasal breathing.
Initial anatomical features (“Before” photos)
When analyzing the face in the frontal projection before surgery, I noted several complex structural abnormalities:
- Deviation of the nasal axis: a pronounced deviation (displacement) of the osteocartilaginous vault to the side was observed. The nose had a pronounced C-shape, making the middle third of the face look asymmetrical.
- Nasal tip: due to the severe displacement of the internal supporting structures, the nasal tip also lost its central position. It looked uneven, shifted relative to the facial midline, and excessively wide.
- Nasal breathing: such a severe external deformity of the longitudinal nasal axis is always associated with a severe septal deviation. The patient confirmed that one half of her nose was almost completely excluded from the breathing process, causing constant discomfort.
Surgical process and objectives
My main goal was to return the nose to a strictly central position, restore perfect facial symmetry en face, and completely normalize nasal breathing, while making the nasal lines more refined and feminine.
Correcting such asymmetry is always high-precision architectural work. First, I performed a septoplasty, completely reconstructing the deviated nasal septum, as it was what created the incorrect “axis” for the external nose. Then, using a delicate bilateral osteotomy, I mobilized the displaced bone structures and set the bony pyramid exactly in the center of the face. The cartilaginous framework of the nasal tip was completely reassembled, freed from asymmetry, and reinforced with support grafts to prevent the tissues from returning to their previous crooked position due to “muscle memory”.
Surgical outcome (“After” photos)
In the “After” en face photos, we see a radical transformation and the return of the face to absolute harmony:
- The nose assumed a flawlessly straight, central position. The C-shaped deviation was completely eliminated.
- The nasal dorsum acquired a uniform, beautiful width and clear, smooth longitudinal lines.
- The nasal tip became symmetrical, centered, neater, and more chiseled.
Thanks to the elimination of the deviation, the patient’s facial features instantly became balanced, her gaze became more centered, and the overall perception of her face became soft and aesthetic. But the main achievement is the full restoration of breathing function: the patient breathes easily and freely through both halves of her nose again and is completely satisfied with the result obtained.
