A young woman consulted me with a classic request to correct a pronounced aesthetic disproportion. Her main source of dissatisfaction was the profile shape of her nose: a large, prominent hump visually weighed down the middle third of her face, making her features overly harsh and depriving her profile of elegance.
Initial Anatomical Findings (Before)
During the profile projection analysis, I noted the following key features:
- Pronounced osteocartilaginous hump: A high, sharp break in the nasal dorsum completely dominated the face and visually elongated it.
- Insufficient tip rotation: The nasal tip pointed slightly downward, making the nasolabial angle overly acute and visually adding age to the face.
- Thick skin: The patient has fairly thick, elastic skin. When working with this tissue type, the surgeon’s task is to build an exceptionally stable and well-defined cartilaginous framework so that the new nasal contours can clearly emerge once the swelling fully subsides.
Surgical Goals and Procedure
My goal was to create a soft, feminine profile while maintaining perfectly natural proportions.
First, I performed a conservative reduction of the bony and cartilaginous dorsum, completely eliminating the massive hump and creating a smooth, refined line. The next crucial step was reshaping the tip: I sculpted the lower lateral cartilages and performed a subtle upward tip rotation. This opened up the nasolabial angle, removed the visual heaviness, and made her overall expression appear more open and welcoming.
Results 3 Weeks Post-Op (After)
In the collage on the right, we see an early result — only 3 weeks have passed since the surgery. For such a short recovery period, healing is progressing excellently. Naturally, at this stage, postoperative swelling remains in the tissues (especially around the tip and supratip area), which will gradually subside over the coming months, revealing an even more refined and elegant nose.
However, significant improvements are already clear:
- The profile is now smooth, fluid, and harmonious.
- The tip has been elevated, eliminating the “parrot beak” appearance and excess length.
- Her overall facial features look considerably softer, highlighting her natural femininity and delicacy.
The patient navigated the early recovery period smoothly, strictly follows all post-op instructions, and is already thrilled with the initial results of her transformation.
