A patient came to me for an initial consultation with a desire to soften her facial features and correct the shape of her nose. The main aesthetic problem was that the nose appeared too long and heavy, which visually added extra age to the face and made the overall expression more stern and tired.
Initial Anatomical Picture (Before surgery)
During the examination and analysis of the facial structure in the three-quarter view, I noted the following anatomical features:
- Nasal dorsum: There was a noticeable bony-cartilaginous hump that disrupted the smoothness of the profile lines. At the same time, the nasal axis had a slight deviation, which often leads to internal deformation of the septal cartilage.
- Nasal tip: Marked age-related ptosis was observed — the tip was significantly drooped downward, and the nasolabial angle was acute (closed). The long, drooping tip visually elongated the central zone of the face.
- Skin and tissues: The patient’s skin is thin, which, on one hand, allows the contours of the new framework to be seen sooner, but on the other hand, requires utmost precision from the surgeon, as even the slightest imperfection is visible on thin skin.
Surgical Progress and Goals
My task was to perform a combined rhinoseptoplasty aimed at softening the facial proportions and restoring its natural youthfulness.
During the operation, I performed:
- Functional stage: Correction of the nasal septum to restore a straight axis and ensure easy, stable breathing.
- Work on the dorsum: Careful removal of the hump and delicate narrowing of the bone structures. I formed a perfectly straight, neat nasal dorsum line.
- Modeling and tip lift: I shortened the terminal part of the nose, performed derotation (raised the nasal tip upward), and securely fixed it in the new position. This allowed opening the nasolabial angle and removing the effect of a “drooping” nose.
Result After 3 Weeks
The “After” photos show a very early result — just 3 weeks after the surgery. During this period, a pronounced postoperative swelling still persists (especially in the area of the nasal tip and the bridge of the nose), which will gradually subside over the course of a year.
However, even at this early stage, the changes are obvious and radical:
- The hump is completely eliminated, and the line of the dorsum has become straight and elegant.
- The nasal tip has been lifted, making the nose look visually shorter and neater.
- Due to the change in the shape of the nose, the patient’s face immediately “opened up,” looking significantly younger, softer, and fresher.
Despite the natural swelling, the patient is already very pleased with the interim result and notes that breathing has become much easier. We will see the full refinement of the lines closer to 6–12 months.
