A middle-aged man presented with complaints of combined problems: severe nasal breathing difficulties and a pronounced aesthetic deformity of the nose shape. The large, hooked nose disrupted overall facial proportions, and the internal deviation of the septum significantly reduced his quality of life, depriving him of the ability to breathe normally.
Initial anatomical features (Before)
During the examination and analysis of the patient’s photographs, I identified key issues that required surgical intervention:
- Nasal bridge: Massive, high bony-cartilaginous hump forming a sharp “hawk” profile. In the three-quarter view, a strong deviation of the nasal axis to the side and a pronounced asymmetry of the bridge are clearly visible.
- Nasal tip: Excessively long, drooping deeply downward. Due to the weakness of the cartilaginous support, the tip aggravated the visual length of the nose and gave the face a heavy, overly stern expression.
- Functional status: Severe deviation of the nasal septum with structural disruption of the internal nasal anatomy, which almost completely blocked adequate airflow.
Surgical procedure and objectives of the surgeon
The specifics of male rhinoplasty require utmost caution from the surgeon: an excessive curvature of the nasal bridge or an over-rotation of the tip upward (an “upturned” effect) are strictly unacceptable. My goal was to eliminate the defects while preserving the strong-willed, masculine character of the face.
As the first and most important step, I performed an extensive reconstructive septoplasty. I straightened the nasal septum and restored the symmetry of the internal airways. Next, I proceeded to aesthetic modeling: I gently resected (removed) the hump, narrowed and centered the nasal bridge, returning it to a strictly midline position. The nasal tip was shortened and fixed in an anatomically correct, natural position for men — at a right angle to the upper lip.
Surgical results (After)
The result demonstrates a complete transformation of the face while maintaining its absolute naturalness:
- Profile: The nasal bridge has become smooth and strong. The former hook-like shape is gone, and the profile lines now look harmonious and refined.
- Nasal tip: The excess length has been eliminated, the tip is elevated to the physiological norm, and the effect that weighed down the face has completely disappeared.
- Symmetry: The nose has taken its correct central position; asymmetry in frontal and three-quarter views has been eliminated.
The patient received not only an aesthetically precise, truly masculine nose that perfectly harmonizes with his facial features, but also, most importantly, fully restored complete, free nasal breathing.
