A young patient came to my consultation with a strong desire to harmonize her facial proportions. The main reason for her dissatisfaction was a pronounced, sharp dorsal hump that dominated her appearance and deprived her profile of femininity. In addition to aesthetic discomfort, the young woman complained of constant nasal congestion—a classic consequence of the deviation of the internal architecture and nasal septum, requiring not just plastic surgery, but a complete functional reconstruction.
Initial anatomical picture (Before surgery):
- Profile: A large, high osteocartilaginous hump with a sharp transition was visualized. The nasal tip was droopy, forming an acute, pinched nasolabial angle. This structure gave the face an overly strict, harsh, and visually older appearance.
- Full-face and three-quarter view: The nasal dorsum appeared wide, with clear asymmetry and axial deviation in the middle regions. The nasal tip was bulbous, poorly defined, and asymmetrical.
- Tissue characteristics: The patient had fairly thin skin. For a surgeon, this means the highest degree of responsibility: thin tissues, like a thin veil, instantly expose any underlying irregularities of the framework. This required extremely smooth, filigree work with bone and cartilage structures.
Surgical course and objectives:
I set a goal to completely eliminate the “predatory” facial expression, making the dorsum straight and the tip neat and stable, while restoring free nasal breathing.
I performed a closed structural rhinoseptoplasty. First and foremost, I carried out a delicate resection of the osteocartilaginous hump and performed a precise osteotomy to narrow the dorsum and return it to the central position. Then, I completely reconstructed the cartilages of the nasal tip: strengthened them, narrowed them, and performed a slight upward rotation, fixing them in an anatomically correct, slightly elevated position. The internal stage was septoplasty, aimed at straightening the septum and widening the nasal passages.
Result (After surgery):
At the follow-up examination, we achieved exactly the result we were aiming for—a natural, chiseled nose that softly highlighted the patient’s natural beauty:
- Profile: The high hump is completely eliminated. The nasal dorsum has become absolutely straight, elegant, and smooth.
- Nasal tip: It has been elevated, gaining clear definition and a beautiful projection. The change in the nasolabial angle instantly “opened up” the face, making it look visually much younger, softer, and gentler.
- Full-face: The dorsum looks neat and narrow, symmetrical dorsal aesthetic lines have been restored, and the tip has acquired a correct triangular shape (which is perfectly visible on the basal projection).
- Function: The septum is straightened, and the patient notes that the quality of her life has changed dramatically thanks to free and easy breathing.
The young woman’s face has been completely transformed while preserving her individuality, and her happy smile in the “after” photos is the best confirmation of the successfully performed work.
