A patient approached me who had long felt self-conscious about the shape of her nose. In addition to obvious aesthetic imperfections, the examination revealed that she was also troubled by difficult nasal breathing — a consequence of a deviated internal septum.
Initial Anatomy and “Before” Analysis
During a detailed analysis of the facial proportions, I identified a complex of anatomical features that required radical yet delicate changes:
- Profile: A massive, high osteocartilaginous hump combined with a long, severely drooping (“diving”) tip. An acute nasolabial angle visually weighed down the lower third of the face and made the expression look more frowning and tired.
- Front view: Noticeable asymmetry in the area of the nasal bridge, as well as a wide tip lacking definition.
- Skin and structures: The skin was of moderate density, but due to a severe excess of cartilaginous and bone tissues, the nose looked disproportionately large for the young woman’s delicate facial features.
Surgical Objectives and Steps of the Operation
My main goal was to give the patient a graceful, natural-looking nose that would emphasize her natural beauty and fully restore respiratory functionality.
I began the operation with the delicate removal of the osteocartilaginous hump, carefully modeling a new, perfectly straight and refined dorsal line. The most critical stage was the work on the nasal tip: I reconstructed and narrowed the alar cartilages, performed rotation (lifted the tip upwards), and securely fixed it in its new position. This made it possible to permanently resolve the issue of the “diving” tip when smiling. Simultaneously, I performed a septoplasty, straightening the septum and clearing the airways.

After 10 months, when the post-operative swelling has almost completely subsided, we can see a stunning transformation:
- The profile line has become flawlessly straight, and the graceful, neat tip has formed a correct, aesthetically pleasing nasolabial angle.
- In the front view, the nose has gained the desired symmetry, clear dorsal lines, and refinement.
- The patient’s face instantly “opened up” — her gaze became softer, the former severity vanished, and a pronounced effect of rejuvenation and lightness appeared.
The patient breathes fully and is absolutely happy, and for me, her self-confidence is the main result of the work done.
