The patient consulted me to undergo an aesthetic and functional enhancement of her nose. During the initial examination and facial proportion analysis, I immediately noted that the original nasal architecture deprived the young woman’s features of their proper refinement.
The main concerns we discussed during the consultation were:
- Excessive volume in the nasal tip area: it appeared overly wide, bulky, and slightly droopy, which visually weighed down the middle third of the face.
- Deformity of the dorsum and nasal axis: asymmetry caused by a deviated nasal septum was clearly visible in the frontal photos. The uneven contour of the bridge visually distorted the entire central axis of the face.
- Impaired nasal breathing: the pronounced deviation of the internal septum predictably manifested as chronic nasal congestion, so the procedure was initially planned as a simultaneous one — rhinoseptoplasty.
An additional complexity in this case was the patient’s thick skin structure. Working with thick skin always requires surgical precision from the surgeon: for a new, refined framework to show through it, the underlying cartilaginous structures must be meticulously sculpted while avoiding excessive scarring.
Surgical Procedure and Outcome
During the surgery, I performed a comprehensive reconstruction. First, a septoplasty was carried out to fully restore free nasal breathing and align the nasal axis. Then, I proceeded to the aesthetic stage: gently narrowing the nasal bridge, eliminating the asymmetry, and completely reshaping the tip. I reduced its projection, rotated it upwards, and gave it a well-defined, neat definition.
At the follow-up examination 6 months after the surgery, we can see an excellent intermediate result. The nose has become neat, symmetrical, and completely harmonious. The elegant slope of the dorsum and the finely defined tip highlighted the patient’s natural beauty, making her gaze more open and her overall look softer and more refined. The patient is completely satisfied with both the aesthetic transformation and the quality of her new, unobstructed breathing.
