Structural Rhinoplasty: Creating a Smooth Profile on Thin Skin (6 Months)

A young woman visited my clinic seeking aesthetic nose correction. She was dissatisfied with her profile: a prominent hump and a drooping tip visually elongated her face, giving it an excessively stern and harsh expression. The patient dreamed of a harmonious, neat nose that would emphasize her natural femininity and grace.

Baseline Anatomical Features (Before)

When analyzing the face in profile and three-quarter view, I highlighted the following key points:

• Nasal bridge: Prominent, high bony-cartilaginous hump creating a sharp, broken profile line.
• Nasal tip: Drooping downward, resulting in an acute nasolabial angle. The tip lacked projection and neat definition, visually weighing down the entire lower third of the face.
• Skin quality: The patient’s skin is quite thin and elastic. On one hand, this is an ideal condition to achieve a beautiful, refined post-operative result. On the other hand, thin skin requires absolute perfection and precision from the surgeon, as it does not forgive even the slightest irregularities on the reconstructed framework.

Surgical Approach and Goals

My task was to completely eliminate the hump, reduce the nose length, and elevate the tip to create a smooth, elegant female profile. At the same time, it was crucial to maintain structural stability so that the shape remains flawless for years to come.

During the procedure, I carefully lowered the nasal bridge, completely eliminating the bony and cartilaginous excess and creating a perfectly straight line. Next, I focused on the nasal tip: performed its rotation (elevated it upward) and secured the alar cartilages in a new, anatomically correct position. This allowed us to open the nasolabial angle and give the tip a refined, neat projection.

Results at 6 Months (After)

The photos demonstrate a stable result six months after the surgical intervention. By this time, the primary swelling has subsided, allowing us to evaluate the final contours:

• Ideal profile: The hump is completely eliminated, and the bridge line has become perfectly straight and natural.
• Harmonious proportions: The nose appears visually shorter, the tip is neatly elevated and has gained beautiful definition.
• Aesthetic outcome: The patient’s face has been fundamentally transformed. Features became incredibly soft, gentle, and refined, and her expression opened up.

The result looks so natural that it fully harmonizes with the patient’s features without revealing any traces of surgical work. The patient is happy, feels confident, and gladly shares her transformation.
