A young woman consulted me seeking to reshape her nose and achieve a softer, more harmonious profile. The primary source of her aesthetic discomfort was a large osteocartilaginous hump and excessive nasal length, which visually weighed down the central part of her face.

Initial Anatomical Findings (Preoperative)

Upon detailed analysis of the patient’s profile, I identified several anatomical features that disrupted facial balance:

  • Nasal bridge: Presenting a high, sharply defined hump that disrupted the smooth contour of the profile line.
  • Nasal tip: Noticeably drooping, resulting in an acute nasolabial angle. This “drooping” tip visually lengthened the nose, adding unwanted sternness, slight fatigue, and extra age to her facial expression.
  • Anatomical framework: The cartilages of the nasal tip required significant structural reshaping to establish an elegant, yet stable and reliable support.

My key objective was to remove the hump, harmonize the nasal length, and subtly lift the tip, leaving the patient’s face looking more open, refreshed, and youthful.

Surgical Procedure and Nuances

During rhinoseptoplasty, I performed a controlled and precise resection of the osteocartilaginous excess along the nasal bridge, completely eliminating the hump and creating a smooth, refined profile line with a natural transition from the radix.

The primary focus was on modeling the tip. I performed a rotation of the alar cartilages to delicately elevate the nasal tip and open the nasolabial angle to ideal feminine proportions. To secure the result for years to come and eliminate any risk of tip ptosis caused by gravity and active facial expressions, I reinforced its structural frame using cartilage autografts.

Result After 6 Months

The postoperative photo on the right demonstrates the outcome 6 months after surgery. Most of the postoperative edema has resolved, allowing us to clearly evaluate the new facial architectonics.

The patient’s profile has undergone a dramatic transformation: the high hump is completely gone, replaced by a flawlessly straight, elegant bridge. The nasal tip is now refined, sculpted, and positioned in its correct anatomical alignment. Notice how much softer and gentler her overall features appear — through the harmonization of nasal proportions, her gaze seems deeper, and her entire look has become remarkably light and sophisticated. The patient is thoroughly delighted with the result, which is the ultimate indicator of a successful surgery.
