Rhinoseptoplasty: A Miniature Nose with Thick Skin

A young woman scheduled a consultation requesting a combined aesthetic and functional correction. She was dissatisfied with a pronounced hump, excessive nasal length, and a drooping tip, which visually weighed down her face and gave it an overly strict appearance. Furthermore, a slight asymmetry of the bridge indicated a deviated internal septum, which caused associated difficulties with proper nasal breathing.

Initial Anatomical Features (Before)

During a detailed analysis of the facial proportions, I noted the following features:

  • Nasal bridge: High, pronounced osteocartilaginous hump. In the frontal view, the bridge appeared flattened and slightly deviated from the central axis of the face.
  • Nasal tip: Massive, drooping downwards, lacking clear projection and definition. Such a tip visually elongated the nose and concealed the natural softness of other facial features.
  • Skin quality: Thick skin in the nasal tip area. For a surgeon, this is always a challenge: thick skin contracts reluctantly and can hide the meticulous work done on the cartilages, so highly precise and structural framework modeling was required here.
Surgical Procedure and Objectives

My primary goal was to restore facial balance by making the nose more miniature, neat, and feminine, but without the “operated” face look.

First, I performed a septoplasty, straightening the septum and restoring free airflow. Then, I proceeded to the aesthetic stage: gently removing the hump, narrowing, and straightening the nasal bridge. The most painstaking work involved the tip — I altered the shape and position of the alar cartilages, lifted the tip, reduced its volume, and gave it a stable, beautiful projection.

Result after 1.5 Months (After)

The photo shows the result just one and a half months after the surgery. Despite the fact that dense postoperative swelling is characteristic for this period (especially on the tip with thick skin), the difference is colossal:

  • The profile became absolutely straight, smooth, and refined.
  • The nasal tip lifted, the former heaviness disappeared, and the nose became visually shorter and more elegant.
  • In the frontal view, symmetry was restored, and the bridge and tip acquired clear, beautiful lines.

The new nose fits perfectly into the overall proportions, beautifully accentuating the patient’s beautiful, expressive eyes and lips. The facial features softened, appearing fresher and gentler. Breathing is completely restored, and my patient is sincerely rejoicing at her new reflection in the mirror.
