A patient came to my consultation with a complex issue — a pronounced aesthetic facial imbalance and severe difficulties with nasal breathing. This was a classic case where external deformation directly reflected a severe internal anatomical impairment.
Initial anatomical features (“Before” photos)
During the examination of the patient in different projections, I identified several complex tasks that required a simultaneous solution:
- Asymmetry and nasal axis (frontal view): a severe deviation of the entire nasal pyramid with a shift to the side was clearly visible. Due to this pronounced deviation, the proportions and symmetry of the entire face were disrupted, and the nasal tip cartilages were positioned unevenly.
- Profile line: a large, high osteocartilaginous hump was present.
- Nasal tip: was droopy, which visually weighed down the face, made the profile look harsh, and added age.
- Functional status: severe nasal septal deviation almost completely blocked the passage of air on one side. The patient had been unable to breathe normally for years.
Surgical course and strategy
As a surgeon, this case was of particular professional interest to me because it demanded meticulous precision. The work was carried out simultaneously in both functional and aesthetic directions.
First, I performed a thorough septoplasty: I corrected and reinforced the nasal septum, returning it to a strictly central position and opening up the airways. Then, I proceeded to the aesthetic stage. I carefully reduced the massive hump, aligned the nasal bridge along the facial midline, eliminating the asymmetry, and performed rotation of the nasal tip. The tip was delicately lifted, narrowed, and secured in its new, harmonious position.
Intermediate result at 3 weeks (“After” photos)
The “After” photos capture a very early postoperative period — only 3 weeks after the surgery. At this stage, a pronounced, dense postoperative swelling is still present on the face (especially in the areas of the nasal tip and the bridge of the nose), which will gradually subside over the course of a year, making the lines even more refined.
Nevertheless, the scope of the work done is already evident:
- The nose became perfectly straight, the deviation was eliminated, and the axis was centered along the nasal dorsum.
- The hump is completely gone, giving way to a noble, straight profile.
- The nasal tip was lifted, opening the nasolabial angle, which instantly made the facial expression look gentle, soft, and rejuvenated.
The patient is incredibly happy: besides the stunning aesthetic transformation, her free and easy nasal breathing has been fully restored. A rehabilitation period lies ahead, but it is already clear that we have achieved a flawless balance of health and beauty.
