Introduction and primary complaints
A charming young woman came to the clinic, and at first glance, the main problem became obvious — a pronounced nasal asymmetry. As a surgeon, I understand that such an aesthetic defect almost always conceals a serious functional problem. And this case was no exception.
Initial anatomical presentation
Having thoroughly studied the patient’s baseline anatomical features “before”, I noted several complex issues at once. Firstly, a severe deviation of the nasal axis and septal deviation, which is clearly visible by the asymmetry of the nostrils and the displaced columella. This structure inevitably impeded nasal breathing, depriving the patient of comfort in daily life. Secondly, a noticeable hump was visible in profile, and the nasal tip was slightly wide and droopy, which visually weighed down the face and gave it a slightly tired look.
Course of the operation and surgical nuances
To address all these tasks, I chose the technique of open hybrid rhinoseptoplasty. It was a meticulous job that required a jewelry-like precision approach. Working with pronounced asymmetry is always challenging: I had not only to “smooth out” the shapes but also to completely reassemble the internal framework, center the septum, and build a reliable support. The nasal tip required special attention — I needed to delicately model the cartilages to make it refined, structural, and harmonious, taking into account the tissue density of the patient.
Outcome: Harmony and functionality
The result, which you can see in the “after” shots right on the operating table, completely lived up to our expectations. I carefully removed the hump, forming a beautiful, smooth transition to the nasal bridge. The nasal axis is now perfectly straight — we have completely overcome the asymmetry. The nasal tip is delicately lifted and narrowed, which made the profile incredibly feminine and soft.
But my main achievement in this operation is that along with the aesthetic harmony of the face, we restored the patient’s free, full breathing. The young woman’s face immediately looked fresher and younger. She was absolutely thrilled with the result, and for me, this is the main indicator of a successfully performed job.
