Initial Consultation and Main Complaints
A young, beautiful girl came in for a consultation. Her eyes immediately drew attention, but the shape of her nose disrupted the overall facial harmony. The main aesthetic concern she presented with was pronounced asymmetry: the nasal axis was noticeably deviated, which inevitably led to a distinct asymmetry of the nostrils. In addition, a prominent hump weighed down the profile, and the nasal tip was quite wide, lacking refinement, and slightly drooped.
Baseline Anatomical Findings
Upon close examination, I noted that such a severe deviation of the external nose is almost always accompanied by a profound deformity of the nasal septum. This meant that the patient definitely experienced difficulties with nasal breathing. Therefore, my task involved not only a visual transformation but also a complete functional reconstruction. An additional nuance was the moderately thick, edema-prone skin. In rhinoplasty, such baseline characteristics always require me to create an exceptionally strong and precisely tailored cartilaginous framework — this is the only way to guarantee that the new nasal tip will be structural and will not lose its shape under the weight of the skin envelope.
Surgical Course and Nuances
During the surgery, I performed a complex functional rhinoseptoplasty. Working with pronounced asymmetry is always a challenge because tissues possess a so-called “shape memory.” I meticulously straightened the septum, opening the airways, and delicately lowered the nasal bridge, completely removing the hump. I paid special attention to the tip: it was carefully narrowed, rotated to a physiologically correct, feminine angle, and securely fixed.
Outcome: Harmony and Functionality
Evaluating the “After” result, I can proudly say that we achieved our goals. We obtained excellent symmetry: the nasal axis became straight, and the nostrils acquired a proper, symmetrical shape. The profile became soft, straight, and refined. The patient’s face was instantly transformed — it began to look lighter, fresher, and much more harmonious, while the heavy, tired appearance became a thing of the past. The young lady is incredibly pleased with the result and can now breathe freely. As our lighthearted post-op joke goes: “the patient has her mother’s eyes, but her perfect nose is from me.”
