Harmonization of a Female Profile with Thin Skin

A young woman sought consultation with a classic request to address facial aesthetic imbalance. Her primary concerns were a prominent dorsal hump and a drooping nasal tip, which gave her profile a harsh appearance and rendered her overall facial expression overly stern. Additionally, during the examination, we revealed a deviated septum that periodically obstructed free breathing.

My task as a surgeon was to perform a delicate harmonization: remove the pronounced hump, elevate the nasal tip, and create a smooth, elegant dorsal contour that would accentuate the patient’s natural beauty and youthfulness.

Anatomical “Before” Analysis:
  • Nasal dorsum: A pronounced, sharp osteocartilaginous hump was identified, disrupting the smooth lines of the profile. In the three-quarter view, the nose appeared excessively bulky.
  • Nasal tip: Drooping downward with insufficient projection. An acute nasolabial angle visually elongated the central part of the face and stripped it of softness.
  • Skin and structures: The patient’s skin is quite thin, which on one hand allows the contours of the new shape to become visible faster, but on the other hand requires surgical precision, as every micron of cartilage structure is noticeable under thin skin.
Surgical Objectives and Procedure:

The surgery was performed using a closed technique, without external incisions. During the functional stage, I corrected the nasal septum, ensuring ideal airflow. I then proceeded to the aesthetic stage: carefully resected the hump and performed an osteotomy to narrow the nasal dorsum.

I paid special attention to the nasal tip: gently elevated (rotated) it, forming a beautiful, open nasolabial angle, and securely fixed cartilage grafts for long-term result stability.

“After” Result (just 3 weeks post-op):

The photographs document a very early result — only 3 weeks have passed. Residual post-operative swelling persists on the dorsum and especially at the tip, which will gradually subside over the coming months.

However, key changes are already apparent:

  • The profile has become impeccably smooth, elegant, and soft.
  • The nasal tip has been elevated, eliminating the “tired” facial look.
  • By reducing the overall nasal length, the facial features appear significantly softer, more harmonious, and younger.
  • Comfortable nasal breathing has been completely restored.

Even though the nose has yet to undergo its final maturation process, my patient is already thrilled with her new reflection in the mirror, and her confident smile is the best reward for my work.
