Facial Harmonization and Correction of a Wide Nasal Dorsum

A classic and highly interesting case — a young woman came to me for a consultation. The patient’s primary desire was to soften her facial features, remove the sharpness of the profile, and make her nose more refined, feminine, and proportional.

Initial Anatomical Baseline (Before)

When analyzing the face from the front, three-quarters, and profile views, I identified the following key features:

  • Nasal dorsum: A pronounced, high osteocartilaginous hump. In the profile and three-quarters views, it was noticeable that the nasal axis had a slight asymmetry, and the dorsum itself appeared excessively wide for the girl’s petite facial features.
  • Nasal tip: Quite long, drooping downward when smiling or talking. The lack of definition and slight drooping of the tip visually elongated the middle third of the face, giving it an overly strict appearance.
  • Functional condition: Deviation of the external nose was combined with a septal deformity, which predictably caused the patient issues with full nasal breathing.
Surgical Progression and Objectives

My goal was not simply to reduce the size of the nose, but to achieve complete facial harmonization while preserving her unique individuality.

I performed a combined rhinoseptoplasty. First and foremost, I straightened the internal septum so that the patient could forget about nose drops and congestion forever. Then, I carefully lowered the nasal dorsum, completely eliminating the hump and narrowing the bone structures to create a refined, beautiful silhouette from the front view. I paid special attention to the nasal tip: I reduced its projection (length) and performed a delicate upward rotation. This made it possible to create an elegant yet absolutely natural nasolabial angle.

Result After 6 Months

After six months, we achieved a stable, harmonious, and very gentle result. The tissues have contracted well, revealing the surgeon’s fine work.

The patient’s profile became flawless — straight, smooth, without a trace of the former heaviness. Due to the shortening of the nose and the delicate lifting of the tip, the young woman’s gaze became more open, and her entire face acquired a soft, pleasant, and refreshed expression. The new shape of the nose perfectly highlighted the beauty of the patient’s eyes and lips. Nasal breathing has been fully restored, and for me as a physician, the highest reward is seeing how much more confident and happier my patient has become.
