A wonderful young woman came for an initial consultation with a history that, unfortunately, is not uncommon in my aesthetic practice. She required a secondary (revision) rhinoplasty. The previous surgical intervention left behind not only aesthetic dissatisfaction but also noticeable psychological discomfort, so it was extremely important for me to restore the patient’s confidence in her own beauty.
Initial Anatomical Features
Studying the initial “Before” picture in detail, I noticed a number of complex anatomical nuances. First of all, it was a rather thick, porous skin. Secondly, the nasal tip looked heavy, asymmetrical, and devoid of clear contours — a typical consequence of scarring processes and the loss of natural cartilage support after the first surgery. The nasal dorsum also had visual irregularities, and a septal deviation directly indicated that, in addition to aesthetic issues, the patient experienced serious difficulties with proper nasal breathing.
Surgical Process
Performing a revision rhinoplasty is always a challenge and delicate work. My main task was to carefully bypass the dense scar tissue and rebuild a reliable, anatomically correct framework of the nose. The patient’s thick skin required a special, extremely delicate approach: to make the nasal tip elegant and structured under such a thick “cover”, I had to create a flawlessly rigid and filigree cartilage support. In parallel, I performed a septoplasty to solve the breathing problem once and for all.
Result
Evaluating the “After” result, I can proudly say that we managed to achieve complete harmony. I created a straight, noble line of the dorsum with a smooth, aesthetic transition to the new nasal tip — now it is neat, symmetrical, and slightly elevated, which visually removed fatigue from the face and made the appearance significantly fresher and younger. The functionality of the nose has been restored one hundred percent: the patient breathes freely with a full chest. The most valuable thing for me as a surgeon is to see the sincere joy of my patient, who has finally found that very ideal profile she had been dreaming of for so long.
