A young female patient sought consultation at our clinic with a clear aesthetic goal: she wanted to make her profile softer, more feminine, and refined. Excess volume in the area of the nasal dorsum disrupted proportional balance, visually weighing down the middle third of the face. In addition, examination confirmed her functional complaints regarding periodic nasal breathing difficulty caused by a deviated cartilaginous septum.
Anatomical Analysis “Before” Surgery
When analyzing the patient’s baseline profile anatomy, I highlighted the following key points:
- Nasal dorsum: Presence of a moderate yet distinct osteo-cartilaginous hump, creating a harsh, broken line and depriving the profile of elegance.
- Nasal tip: Had insufficient projection and was slightly drooping downward, making the nasolabial angle overly acute.
- Skin quality: The patient’s skin thickness leaned toward thin. Working with thin skin is always a true test of surgical mastery. Thin dermis perfectly highlights new, refined contours, but it completely fails to conceal even the slightest imperfections in bone or cartilage work. Every movement on my part had to be extremely precise and calibrated.
Surgical Plan and Procedure
My main goal was to reduce the dominant nasal volume, restoring balance and natural delicacy to the patient’s face while avoiding an “operated-on” look.
During the surgery, I performed a careful and conservative reduction of the nasal dorsum, completely leveling the hump and creating an impeccably smooth, noble line. The next critical step was refining the nasal tip: I performed delicate tip rotation (lifting it slightly upward) and subtly adjusted its projection. This opened the nasolabial angle to harmonious feminine proportions. As an internal step, I performed a septoplasty, fully correcting the architecture of the nasal septum and restoring clear breathing.
