A young female patient scheduled surgery with me to narrow a wide nasal bridge and restore her breathing. She was dissatisfied with the excessive width of her nose, which stripped her facial features of their natural femininity, as well as a constant feeling of nasal congestion.
Initial Anatomical Presentation (“Before” photo)
- Bridge and axis: Noticeable widening of the osteocartilaginous vault of the nasal bridge, making it look flat and ill-defined from the front view. There was a slight visual asymmetry of the nasal axis caused by a deviated septum.
- Nasal tip: It was bulbous, heavy, and lacking a clear structure.
- Skin characteristics: The situation was complicated by the patient’s rather thick skin. Working with this skin type requires rigid and precise structural modeling of the cartilaginous framework by the surgeon, otherwise the dense tissues will simply “hide” all the changes.
Surgical Procedure
My goal was to narrow the nose, refine its contours into more delicate and defined lines, and fully restore free breathing.
During the surgery, I performed a careful osteotomy to narrow the bony base and nasal bridge, and corrected the internal septal deviation. Particular attention was paid to the tip plasty: I excised the excess lateral cartilage and brought them closer together, shaping a neat, stable, and graceful projection.
Result at 7 Months
In the photo, we see the result 7 months post-operation. For thick skin, this is still an intermediate period, and the process of tissue contraction is still ongoing, but a colossal difference is already visible.
The nasal bridge has acquired clear, elegant aesthetic lines, and the tip has become refined, neat, and defined. The new shape of the nose harmonized the proportions, making the patient’s face softer, more open, and refined. Nasal breathing has been fully restored, and the young woman feels wonderful.
