A young man came to my consultation with pronounced aesthetic and functional defects resulting from a severe deviation of the osteochondral structures. The main feature of his case was a severe post-traumatic or congenital deviation, which not only disrupted facial harmony but also critically blocked free nasal breathing.
Initial anatomical picture (Before)
Upon examination and analysis of the photographs in full face, profile, and basal projection, I recorded a complex of severe defects:
- Pronounced asymmetry and S-shaped deviation: The nasal axis was severely shifted to the side, and the dorsum appeared deformed and uneven in width.
- Massive hump and drooping tip: In profile, a large cartilaginous and bony hump stood out clearly, and the nasal tip pointed downward, visually weighing down the middle third of the face.
- Basal defect: The bottom view clearly shows a severe collapse of the alar cartilages and a sharp deviation of the septum, which almost completely blocked one of the nasal passages.
- Thick skin: The patient has fairly thick skin, which always requires jewelry precision from the surgeon when working with the underlying framework — the structures must be shaped so that the result is “readable” even through a thick skin flap.
Surgery progress and surgical objectives
My work in this case was comprehensive and reconstructive. The first and most important stage was a deep septoplasty: I completely straightened and strengthened the nasal septum, restoring the symmetry of the nasal passages and opening the way for normal breathing.
Then I proceeded to aesthetic modeling. I needed to carefully eliminate the massive hump while preserving the masculine character of the profile — for men, it is extremely important to avoid excessive scooping of the dorsum. With utmost delicacy, I worked on the tip section: the cartilages of the tip were narrowed, rotated to the correct anatomical angle, and securely fixed in a new, symmetrical position.
Result after 6 months (After)
At the follow-up examination six months later, we see an excellent, stable result. The nose has become perfectly straight, centered along the midline of the face.
We managed to:
- Completely eliminate the hump, shaping a straight, noble male nasal dorsum.
- Correct and narrow the nasal tip, making it neat and proportional.
- Achieve perfect symmetry of the nostrils and the nasal base (which is perfectly visible in the basal view).
- Most importantly — fully restore full and free nasal breathing to the patient.
The young man’s face began to look more open, harmonious, and attractive, while completely retaining its masculinity and individuality. The patient is incredibly pleased with the quality of his new life and how his reflection in the mirror has changed.
