Introduction and Main Complaints
A young woman came for an initial consultation, the main reason for her visit being a pronounced nasal asymmetry that disrupted the natural harmony of her face. In addition, as is almost always the case with such visual deformities, the patient experienced severe difficulties with nasal breathing, which significantly reduced her quality of life.
Initial Anatomical Picture
During a detailed examination of the initial presentation, I noted a severe deviation of the dorsum and nasal septum — the central axis was noticeably shifted. The nasal tip appeared quite wide, heavy, and asymmetrical, which was visually underscored by the differing shapes of the nostrils. I also noticed the patient’s rather thick and porous skin. For me as a surgeon, this presents a certain challenge: thick skin conceals the underlying definition, and shaping a refined, sculpted tip beneath it requires jeweler-like precision and the creation of a highly reliable cartilaginous framework that will not lose its shape over time.
Course of Operation and Surgical Nuances
I performed an open hybrid rhinoseptoplasty. My task was complex — it required not only creating an aesthetically pleasing profile but also completely reconstructing the internal anatomy. First and foremost, I straightened the nasal septum, eliminating the functional blockage and restoring full, deep breathing to the patient. Then, the meticulous work on aesthetics began: I aligned the nasal dorsum along the correct central axis, narrowed and structurally refined the tip, making it lighter and more delicate, and achieved perfect symmetry of the nostrils.
Outcome: Harmony and Functionality
Evaluating the result in the “After” photo, I see a perfectly straight, harmonious nose that fits ideally into the facial proportions. The features became more refined, and the overall appearance looks fresh and attractive. The patient was absolutely thrilled with the transformation and now breathes freely. As we noted with a smile after the surgery: “The eyes are mom’s, but the nose is from Gevorg Yurievich.” Seeing happy eyes and restored self-confidence is the best reward for my work.
