A charming patient came for a consultation, whose case is a classic example of how severe aesthetic defects are inextricably linked to functional impairment. The young woman was concerned about pronounced facial asymmetry, and after assessing the baseline presentation, I immediately understood the scope of the upcoming work.
Baseline Anatomical Presentation
Upon detailed examination, I diagnosed a complex deformity: severe deviation of the nasal axis and nasal septum. The nasal tip was bulky, lacked structural definition, and was significantly shifted to the side, which led to a pronounced asymmetry of the nostrils. In addition, I noted the patient’s rather thick and porous skin. With such a radical septal deviation, the young woman expectedly experienced serious difficulties with nasal breathing—her quality of life and daily comfort were significantly reduced.
Surgical Course and Technical Nuances
To address this complex set of issues, I performed an open hybrid rhinoseptoplasty. I faced a task that required meticulous precision. Working with thick skin always poses a challenge to a surgeon, as it is reluctant to take on a new shape and tends to conceal the definition. Therefore, it was necessary for me to create the most durable and anatomically precise cartilaginous framework that could define a beautiful, refined contour. But, undoubtedly, the most critical step was the total reconstruction and centering of the nasal septum.
Outcome: Harmony and Functionality
The result of the operation speaks for itself. In the “After” frames, you can see how radically the facial architecture has been transformed. I completely straightened the nasal bridge, brought the tip into an ideal central position, and achieved harmonious symmetry of the nostrils. The nasal tip itself became elegant, refined, and neat, despite the initial tissue thickness. I did not just eliminate the visual asymmetry, but completely restored free physiological breathing. The patient’s face acquired balanced, soft proportions, and she gained the long-awaited confidence in her beauty.
