Structural rhinoplasty: hump reduction on thin skin

A young woman approached me for help in creating a neat and harmonious female nose. Her main aesthetic concern was a pronounced nasal hump, which disrupted facial balance, giving the profile an overly stern appearance and visually adding age. Our goal was to emphasize the patient’s natural beauty.

Initial anatomical picture (Before)

When analyzing the profile and three-quarter views, I identified the following key features:

  • High osteocartilaginous hump: The nasal bridge had a sharp, prominent contour that completely dominated the face and deprived the profile of smoothness.
  • Insufficient tip rotation: The nasal tip was slightly droopy, making the overall length of the nose appear excessive, and the nasolabial angle was overly acute.
  • Skin quality: The patient has thin, delicate skin. For a surgeon, this is always a highly complex challenge: thin skin does not forgive the slightest irregularities of the underlying framework, so the smoothing of bony and cartilaginous structures must be performed with jewelry precision.
Objectives and nuances of the surgery

My primary task was to perform a gentle structural rhinoplasty. I carefully modeled the nasal bridge, removing the rigid bony and cartilaginous hump and shaping a perfectly straight, soft line.

Then I proceeded to work on the tip: I altered the shape of the alar cartilages and slightly lifted (rotated) the nasal tip upward to open up the nasolabial angle and give the face a younger, fresher, and more welcoming look. During the surgery, I also reinforced the internal supportive structures of the nose to guarantee the stability of the aesthetic result and maintain perfect nasal breathing.

Early result at 3 weeks (After)

In the presented photos, we can see an early but already very indicative result — only 3 weeks have passed since the surgery. Certainly, at this stage, post-operative swelling still persists (especially in the nasal tip area), which will gradually subside over several months, making the lines even more defined.

Nevertheless, the difference is colossal:

  • The hump is completely eliminated, and the bridge line has become elegant and feminine.
  • The nasal tip is lifted, and the visual heaviness and excessive length are gone.
  • The overall facial features have become noticeably softer, gentler, and more harmonious.

The patient handled the rehabilitation period excellently and, even at this early stage, is incredibly happy with the ongoing changes. Her new reflection in the mirror brings her confidence, and the nose looks completely natural, harmoniously fitting into the facial proportions.
