A young woman scheduled a consultation with me with a request typical for the Caucasian type of appearance. She was categorically dissatisfied with the shape of her nose in profile: a large hump and a drooped tip made her facial features excessively harsh, visually weighed down the middle third of the face, and added age. In addition, during a detailed examination, it turned out that the patient regularly experienced difficulty in nasal breathing, especially at night.
Initial Anatomical Features (“Before” Photos)
Upon examination of the profile projection, I highlighted the following key points for surgical correction:
- Nasal dorsum: a high, pronounced osteocartilaginous hump was identified, which disrupted the harmony and smoothness of the facial contours.
- Nasal tip: was drooped downwards (beak deformity), making the nasolabial angle acute. This visually lengthened the nose and created an effect of permanent tiredness on the face.
- Anatomy and function: the pronounced external deformity was combined with a deviated nasal septum, which was the root cause of chronic nasal congestion.
Surgical Goals and Procedure Steps
My primary goal as a surgeon was to remove the visual heaviness, make the profile soft and feminine, while completely preserving the patient’s individuality and ethnic charm, avoiding any artificial effect.
I performed a comprehensive rhinoseptoplasty. The first, fundamental step of the procedure was correcting the internal architecture of the nose — I straightened the deviated cartilaginous part of the septum, fully restoring normal airflow. Next, I proceeded to the aesthetic stage: I performed a delicate lowering of the bony and cartilaginous nasal dorsum, completely eliminating the hump. The most crucial step was the rotation of the nasal tip — I gently lifted it, shortened the overall length of the nose, and formed a beautiful, open nasolabial angle.
Surgical Result (“After” Photos)
The postoperative image clearly shows how radically, yet softly, my patient’s appearance has changed:
- Profile: the nasal dorsum became flawlessly straight, with a smooth, natural transition.
- Nasal tip: was lifted, acquiring a well-defined shape and neat contours. The nose no longer appears long.
- Overall impression: the face instantly “opened up,” looking significantly younger, softer, and more harmonious.
The aesthetic result exceeded the patient’s expectations, and most importantly, she can now breathe absolutely freely and fully. The new shape of the nose perfectly highlighted the natural beauty of her eyes and lips, making the look complete.
