A young man approached me with complaints of pronounced nasal asymmetry and a near-total absence of normal nasal breathing. This case is a vivid example of how aesthetic defects are directly intertwined with serious functional impairments.
Initial Anatomical Features (“Before” Photos)
When analyzing the face from the front view, I identified a number of complex structural changes that required large-scale reconstruction:
- Total deviation of the nasal axis: the bony pyramid and the cartilaginous part were severely displaced to the side (pronounced deviation). The nose looked asymmetric and literally “broke” the overall proportions of the face.
- Impairment of respiratory function: notice the patient’s slightly open mouth in the “Before” photo. This forced position is a classic clinical sign that the nasal septum was critically deviated, causing the free airflow to be blocked, and the young man had to constantly breathe through his mouth.
- Nasal tip: due to the severe displacement of the internal structures, the nasal tip also lost its symmetry, was deformed, and shifted relative to the midline.
Surgical Procedure and Surgeon’s Objectives
When working with male rhinoplasty, especially in cases of severe deformities, my main goal is to return the nose to an anatomically correct position, fully restore breathing, but at the same time preserve strong, masculine facial features, avoiding excessive reduction.
The operation was of a large-scale reconstructive nature. I performed a complex septoplasty, restoring the continuity and straight shape of the nasal septum. Using precise osteotomy, I mobilized and repositioned the displaced bony structures, aligning them strictly along the midline. The nasal tip was completely reassembled, centered, and reinforced with special cartilaginous grafts to overcome “tissue memory” and securely fix the new, straight framework.
Surgical Result (“After” Photos)
The result in the “After” photo clearly demonstrates a radical transformation:
- The nose took a strictly central position, the bridge became straight, and facial symmetry was fully restored.
- An important detail: notice how in the photo on the right, the patient’s lips are now naturally and calmly closed. This is the best confirmation that nasal breathing has been completely restored, and the patient has finally gotten rid of constant discomfort.
The facial features became balanced and harmonious, while the nose looks absolutely natural and masculine. The patient gained not only aesthetic confidence but also a new quality of life thanks to free breathing.
